Information and Service Requests
The following forms may be downloaded in Adobe Reader format. Once you download and fill out a form you may either mail it to the police station, fax it (401-423-3710) or hand deliver it.
If you do not have the Adobe Acrobat Reader please download it before attempting to download one of the forms below.
Request a Detail Officer - This form is used for requesting a detail officer. Page 1 explains the wages and benefits Jamestown Police Officers are entitled to. Please complete Page 2 and either fax or hand deliver it to the police station. Please note that your detail officer may be either a police officer or a community service officer.
Police Records Request - Use this form for requesting copies of official police reports.
Home Security Check Request - Use this form to request police officers check your residence while you are away on vacation.
Charitable Gaming Permit - If you would like to hold a game of chance for a charitable cause, please file for a charitable gaming permit with the Rhode Island State Police.
Complaint Form - This form is for filing a general complaint or to report an incident. If you need immediate police assistance please call 401-423-1212, or 911 for emergencies.
Speed Trailer Request - If you would like the RADAR Speed Trailer to be deployed in your neighborhood please fill out this request form.
Civilian Complaint Form - If you would like to file a complaint against a member of the Jamestown Police Department please click on the above link and follow our Civilian Complaint Procedures.
State Accident Report - If you were in a motor vehicle accident and the damages exceeded $1,000, you are required to file a State Accident Report within 14 days of the accident. Failure to do so could result in your driver's license or vehicle registration being suspended. Print this form and follow the instructions on the first page.
